- Proof of hotel reservation or accommodation should be submitted.
- Proof of financial sufficiency that covers the applicant's duration of stay should be submitted.
- A copy of the applicants travels itinerary with flight dates and tickets.

- For private business owners, a copy of their Income Tax Return(ITR) form or certificate of income tax deducted from salary should be submitted.
- Proof of hotel reservation or accommodation that covers applicants' duration of stay should be submitted.
Italy Medical Visa Eligibility & Requirements
- Malians who apply for this visa seek medical attention that cannot be gotten in their country in Italy
- Your local doctor should provide your diagnosis stating the reason treatment is best administered in Italy and the duration of the treatment.
- A letter from the doctor in Italy stating that he has accepted to take up the treatment should be provided. The nature of the treatment and the intended time of recovery should be clearly stated.
- Applicants are to possess a passport that was issued within the last ten years and should have a minimum validity of 6 months from the date of travel.
- Two recent passport photographs not older than 3 months should be submitted.
- Proof of health insurance that has a minimum value of 30,000 euros should be submitted
- If you are employed, a letter from your employer is required. Your employer should state in the letter that he is aware of your leave. Your position and how long you have been employed in the company should be clearly stated.
- Proof of payment of medical fees should be provided.
- If you are under the age of 18 years, the original copy of your birth certificate should be provided.
- Holders of this visa are prohibited from taking up employment in the country.
- You are expected to leave the country at the end of your treatment.
- Holders are not allowed to take up permanent residency in the country.
- Proof of paid visa application fee is required.
