Travelers are required to fulfill the following requirements in order to apply for a Polish Visa.

Poland Business Visa Eligibility & Requirements
- The Business visa is issued to Namibians who will visit Poland for the purpose of business, investment, marketing, commercial transactions and handling corporate affairs.
- Your visa application form must be completely filled.
- A recent passport-sized photograph will be needed to complete this application.
- Invitation letter from the Polish company you will be visiting and their detailed address accompanied with the dates of your visit.
- A certificate from your employer stating/allowing your business travel.
- If there were previous trade relations between the two companies, proof of such events must be provided.
- Business bank statement for the latest 6 months.
- Memorandum and Article of Association in original certified copy (registered with joint-stock companies) Trade License (first issued and present renewal), Proprietorship/Partnership documents.
- Regarding the applicant’s expenses during stay in the Schengen zone, either the employer or the partner company must state coverage of expenses on the letter or invitation.
- For the self-employed, a copy of your business license, company bank statement for the latest 6 months.
Poland Medical Visa Eligibility & Requirements
- Valid travel document/passport. Make sure your passport has a validity of three more months beyond your planned departure from the Schengen territory.
- A copy of your passport’s data page.
- Roundtrip flight reservation. A document that shows details regarding your flight to the Schengen Area and back, including flight numbers and dates.
- Travel Medical Insurance. Covering the whole Schengen Area, with minimal coverage of €30,000 during your whole period of stay.
- Proof of accommodation. This could be any of the following:
- Confirmed Accommodation in the Medical Clinic/Hospital.
- Hotel Booking.
- Letter of invitation.
- Proof of civil status: Marriage certificate, birth certificate of children, death certificate of spouse, if applicable.
- Proof of paid fee. Which is €60.
- Proof of financial means. Proof that the applicant has sufficient financial means to pay the medical expenses and other related expenses throughout their treatment.
- Letter of sponsorship plus bank statements of the sponsor.
- Proof of advance payments for the treatment plus bank statements for other related expenses.
- Verbal note from the Ministry of Health of applicant’s home country. If the ministry of health in your home country will be covering the costs to your health treatment a letter declaring the readiness of the government to cover your medical treatment costs.
- Birth Certificate.
- If you are minor a formal document from you and the other parent of your child, stating they have no objection about your child travelling with a third person, if applicable.
Poland Tourist Visa Eligibility & Requirements
- The tourist visa is for Namibians who intend to travel to Poland for visiting natural beauties and historic places.
- Please be aware that employment is not permitted with a tourist visa.
- You're also not allowed to partake in business activities while in Poland.

- Rent agreement, if you have rented a place, in the country you will be staying.
- Letter of tour organizer. If you will be travelling with a tour agency.
- Evidence that shows you have enough money to support yourself throughout your stay in Poland
- Bank account statement.
- Sponsorship Letter should be presented if someone else will be financially sponsoring your trip to Poland.
