Application requirements
Documents Pakistani applicants need to provide.
- Original Pakistan passport that is valid for at least 6 months after the intended period of stay in Poland.
- Passport must have at least one blank page for the visa stamp.
- Poland application form that is completely filled and signed.
- One recent passport photograph with a white background.
- Copies of tickets or confirmed travel itinerary showing entry and exit date.
- Copies of most recent bank statement showing proof of financial means.
- Copies of confirmed hotel reservation as proof of accommodation in Poland.
- Receipt showing that the application form has been paid.
- A record of good character signed by the local police department in Pakistan is required.
- A certificate of yellow fever vaccination is to be submitted by applicants.
- All applications and supporting documents must be in English Language or Dutch.
- Pets must be accompanied by a veterinary certificate issued by a registered veterinarian from Pakistan.
